Many kinds of races take place in different locations and landmarks around the city. Many of these are very similar to real-life location in Chicago, Illinois, but are modified and scaled to fit the driving proportion of Midtown Madness.

Trivia
- The game's version of the city of Chicago is meant to represent Chicago, Illinois, USA. The city has some landmarks missing and is not entirely accurate as the real city. Some of the differences are as follows:
- The Meigs Airport is now demolished.
- The Picasso Sculpture is not in Daley Plaza.
- The Flamingo sculpture in-game is located right next to Daley Plaza. In real life, it is located a few blocks south.
- Some buildings have been relocated in order to enhance gameplay. [1]
